Assalamu Calaykum,
Greetings and Salutations SomaliNet community members.

This post is about FGM and MGM (Male Genital Mutilation). This topic is controversial and taboo to talk about, especially in the Somali community. What prompted me to start this topic is: some of our countrymen/women are uneducated about their reproductive organs (what they look like intact and their function). Also, parents living in countries that circumcise infant boys must be made aware that a high percentage of baby boys are cut without any anesthesia, or painkillers! For 15 minutes or more (the time varies) the baby boy is strapped down and has one of the most sensitive parts of the body (the prepuce has more than 20,000 nerve endings and is the most sensitive/ pleasurable part of the penis) crushed, ripped, and sliced. Since this procedure is sometimes done behind closed doors without the parents present, parents are generally not aware of the torture, terror and unimaginable pain they are inflicting on their baby boys (and girls where FGM on babies is performered like in Gambia). There are a lot of resources (pictures, diagrams, videos, etc.) on the internet. Also one could find books on human anatomy and genital mutilation.

Addendum

Hundreds, possibly thousands of children around the world die as a result of genital mutilation each year. Also, there are complications and side effects of IGM (Infant Genital Mutilation). A few of the many side effects of Male Infant Genital Mutilation include:

Death
Amputation of Glans (head of penis)
The baby can go into shock, some go into a coma
Severe pain
Loss of the protection of the prepuce (foreskin) from urine (which burns the baby), feces, air, soap, bath water, chafing from diaper or clothing (The Glans was intended to be an internal organ).
The list goes on...


I shall update this thread with sources and statistics and further reading suggestions as well as Quranic verses. This thread covers both FGM and MGM. IGM (Infant Genital Mutilation) and MGM are not talked about as much as FGM. The complications of FGM are more severe. The effects of MGM are pernicious, insidious and have a far-reaching impact not only on the mutilated individual but on society as well.

Some of the side effects and complications of FGM will posted on another reply.

MGM (Circumcision) is recommended by doctors. There are no long term impact. If anything, MGM fights infections.
Maalin Wanaagsan. Yes the topic title is MGM. Rather than start another thread I decided to discuss FGM on this thread as well. Just because an authority figure recommends something doesn't make it right. Not all doctors recommend routine infant genital mutilation. As far as I am aware most Medical Associations in the world do not recommend routine infant circumcision. In fact most of the world's males are not mutilated. There is a vested interest for companies and doctors to perform routine infant genital mutilation. It is a business. Doctors who perform the mutilation make money (sometimes as much as one thousand dollars per procedure). The companies that make the implements used to clamp and cut the prepuce benefit. The company that makes the restraint for the baby's body makes money. And the companies that sell anti-wrinkle creams that have baby prepuce (foreskin) as a constituent benefit.

There are far-reaching impacts to the mutilated individual, and possibly to his spouse as well. The initial trauma of the mutilation is one such impact. This is especially so if no anesthesia is used as is the case in the majority of cases (though the rates are decreasing in the West as doctors are faced with ethical issues). The reasoning for this is doctors for a long time didn't believe a baby could feel pain. They thought a baby boy's nervous system wasn't as developed as older children and adults. A study by Canadian researchers at SickKids found that babies feel and remember circumcision pain:

Babies feel pain more acutely and in a larger part of the body than older children and adults. There is also the Psychological Damage: from this torture, especially if the baby could feel the crushing, ripping and slicing of his member for some fifteen or twenty minutes . There is possible brain damage. Sexual Damage:There is nerve damage with an estimated 20,000 to 100,000 erogeneous nerve endings amputated. One function of the prepuce is self lubrication among the more than 15 Functions of the Foreskin (which I will mention in a subsequent reply). As to how it may effect the male's relations with his wife, painful intercourse and ercectile dysfunction are possible adverse effects of male genital mutilation. Consequently, MGM affects women also. Thanks to videos, diagrams, pictures and crucial information on the internet and other media as well as the work of activists and conscientious parents, doctors and others, routine infant male genital mutilation rates are decreasing in the United States [Source: CDC (Centers for Disease Control and Prevention)].

As for MGM fighting infections this is a fallacy. The warm, moist environment of excreta in the diaper will harm the raw wound of the baby's Glans and is a breeding ground for bacteria until the next diaper change. Every time the baby urinates he experiences the pain of the urine stinging his raw, sensitive and exposed Glans which was meant to be protected by the prepuce. Just because infection is possible doesn't mean it will happen. Amputation of a healthy God given part of an organ is not sound medical science. A routine hygiene regimen protects against infections. Also chemicals in soap and bubble baths and other products have been said to cause urinary tract infections.

Some of the Known Functions of the Prepuce

Size: To facilitate proper erection, Sometimes too much skin is cut off the penis (probably as a result of the individual struggling). This can result in painful erections and/ or curvature of the penis when aroused causing tightness and pain.
Protection: From contaminants, air, clothing/ diaper, etc [which cause keratinization (roughening) of the exposed glans]. Like the eyelid protects the eye, the prepuce protects the glans. The surface of the glans is supposed to be sensitive, moist, warm and soft. It Protects the erogenous nerves and contains an estimated 80% of the erogenous nerves of the penis.
Stores pheromones.
Protects the glans from injury.
Self Cleansing and is part of the Immune System with the soft mucosa of the inner prepuce producing plasma cells, which secrete immunoglobulin antibodies, and antibacterial and antiviral proteins, such as the pathogen-killing enzyme called lysozyme. All of the human mucosa (the linings of the mouth, eyelids, vagina, prepuce and anus) are the body's first line of defense against disease.
The prepuce contains Lymphatic Vessels, which are necessary for proper lymph flow and immunological functioning.
Sebaceous Glands produce oils that moisturize and lubricate the glans.
Dartos Fascia: This is a smooth muscle sheath that underlies the scrotum, the entire penis and the tip of the prepuce. It is necessary for proper temperature regulation of the genitals (causing these structures to elongate in the heat and shrink in the cold). Approximately half of the Dartos fascia is destroyed by the mutilation.
Ridged bands: The inner prepuce contains bands of densely innervated, sexually responsive tissue. They constitute a primary erogenous zone of the human penis and are important for realizing the fullness and intensity of sexual response.
The Frenulum: This is a highly nerve-laden web of tissue that tethers the inner prepuce to the underside of the glans. It is similar to the frenula found under the tongue, the upper lip and the clitoral hood (female prepuce). For many intact men, the penile frenulum is a male "G-spot" that is highly pleasurable when repeatedly stretched and relaxed during sexual activity. Depending on the surgical method used, the frenulum is partially to completely destroyed by the mutilation.
Proper blood flow: The prepuce contains several feet of blood vessels, including the frenular artery and branches of the dorsal artery. The loss of this rich vascularization interrupts normal blood flow to the shaft and glans of the penis, damaging the natural function of the penis and altering its development.
Gliding action: The prepuce is the only moving part of the penis. During any sexual activity, the prepuce and glans work in unison; their mutual interaction creates a complete sexual response. In intercourse, the non-abrasive gliding of the penis in and out of itself within the vagina facilitates smooth and pleasurable intercourse for both partners. Without this gliding action, the corona of the circumcised penis can function as a one-way valve, dragging vaginal lubricants out into the drying air and making artificial lubricants essential for non-painful intercourse.

MGM leads to loss of 50% of penile skin, loss of sensitive tissue, muscles, 80% of erogenous nerves and other complications. This includes botched procedures where the glans (head of the penis)is amputated.
You can't change the form of an organ without affecting the function.

It's true, its very unhygienic if u are a man and not circumcised (Buuryaqab).

Most kids get it within the first month they are born, so u can't say it's hurting the baby, since they wont remember the pain.

LoL I don't know how true its, but Somali girls won't marry u if u aren't circumcised.

If an intact male regularly bathes there will be no problem. When cleaning a baby boy's penis all that is needed is to clean the outside. In infancy and through childhood until puberty, and sometimes past it the prepuce is fused to the glans of the child. No attempt should be made to pull back the prepuce to clean the inside of the organ. The prepuce retracts on its own after time. Some doctors in North America who are ignorant of the intact male organ forcibly pull back the prepuce of the baby during well-baby visits. These doctors themselves might have been mutilated as infants. Great harm, pain and bleeding can result from prematurely pulling back the prepuce. There have been cases where doctors having damaged the baby's organ, suggested mutilation as a solution to this iatrogenic complication. Smegma (baaro is shed skin cells, skin oils and moisture. It is found in males and females. The shed skin cells do not fall away as easily as elsewhere in the body. The prepuce is fused to the glans similar to a fingernail on its nail bed). The dead skin cells will go through the preputial orifice without any manipulation while the prepuce hasn't separated from the glans. Parents are told by doctors to clean the prepuce as one would clean a finger (only the outer skin). Once the prepuce has been separated from the glans, children can be taught to pull back the prepuce when urinating, and to clean the glans with warm water when bathing.

Some males are mutilated the same day that they were born, others a week later, still others some time down the line for whatever reason. For those baby boys who are mutilated the same day that they were born, this is a compounding of the trauma of birth that they have experienced. By saying "they won't remember the pain" you acknowledge the hurting of the baby. One only needs to watch a neonatal genital mutilation video where no anesthesia or pain relief is used on the newborn to understand the horrific trauma that is visited upon these tiny babies. As for the babies not remembering the pain, first, there are studies showing maternal bonding and nursing is adversely affected.

The baby loses trust in the mother especially. Second, there were cases where babies' temperaments changed such as crying intensity and duration well over a year after the mutilation. There have also been cases where babies suddenly started staring into space and ignoring their parents after the mutilation. This is a sign of PTSD (Post Traumatic Stress Disorder). The brain might bury the trauma deep in the subconscious to protect the baby from the memory. So, consciously the baby might not remember, but the subconscious which affects the baby's personality and behavior remembers. The site of the wound remembers as well. The glans will become rough to protect itself from the elements, chafing of clothing, etc. If you read about the function of the prepuce you will appreciate the Sublimity of the Work of the Masterful Engineer who constructed this great part. The prepuce is not just skin. The babies remember but cannot communicate this to their parents. The baby is small and dependent on his parents, but he is a human being with a soul and feelings.

The estimates of Somali people in Somalia who have undergone male or female genital mutilation is near universal with close to a hundred percent. According to the United Nations Children's Fund (UNICEF) Somalia leads the world in FGM per capita percentage with an estimated 98 percent of females between the ages of 15 and 49 mutilated. I think you may be right in that Somali women will not marry an intact man and vice versa.

This thread is for the express purpose of providing urgent information to the community. It is not about passing judgment. I think people can make informed decisions for themselves and their relatives if they view the issue from all angles. It is doubly important when one makes an irreversible decision for someone. I am not against genital mutilation for medical emergencies where all other therapies have failed, but only as a last resort. I think it is harmful to perform a life threatening procedure on someone, especially if they did not consent and no anesthesia was used. The worst type of genital mutilation depending on long term effects, in my opinion, is the excision of the external female genitalia and the sewing together of the raw surfaces. When it comes to the actual procedure, I think babies feel the most pain. I think cliterodectomy and other procedures done on baby girls is probably the most horrendous pain a human being can experience.

The historical origins of genital mutilation, superstitions and myths associated with genital mutilation, and ethical concerns as well as other data shall be forthcoming in subsequent entries inshallah.

Mutilate: To injure, disfigure, or make imperfect by removing or irreparably damaging parts.

For most people, subconscious programming from an early age and cognitive dissonance (the procedure is scary and painful vs. it is a good thing) renders it difficult to give what is being presented a second thought. I should know. For a very long time I did not question or even research what genital mutilation is. I did not question why this ritual is done or read about what the functions of the parts that are cut are. I thought it was necessary and obligatory.

Parents force genital mutilation on their children out of love. They don't want their children ostracized, they want them to get married. It is ironic that out of love, they gamble with the lives of their children and cause them severe pain and terror.

Left to their own devices most people would not allow themselves to be mutilated (especially not without anesthesia and pain relief). The Fight or Flight response, an instinctual survival process would take over. The person would do anything to escape. The proof is the restraint of babies (their limbs are restrained), and the holding down of girls by grown men sometimes (having no concern for the person's dignity or cawro).

I think Somali people are afraid of ridicule to even broach the subject. I have read and seen interviews of Somali women who have lived with the harms of FGM and suffered tremendously say they would do it to their own daughters. When asked why? They replied it was tradition ("it was done to us, our mothers, grandmothers and ancestors."). If you could share with someone information that could potentially save the lives of their children, or prevent serious lifelong complications; would you?

Just because something has been done to the majority of the Somali people does not make it beneficial to do it to others. One strong reason that FGM is practiced is because of economics. In some places a girl who has undergone Pharaonic mutilation commands a high bride price. For those that say it is a religious requirement, take a look at the rates of FGM in Saudi Arabia. It is rare in that country.

Some societies cloister their women, others practice FGM. The societies that practice FGM have their women work outside the house (they cannot afford to keep the females at home), so since they cannot monitor them all the time they cut the girl to make a built in chastity belt.

One cannot fathom the pervasive harm Genital Mutilation has caused to babies and children, as well as men and women (physical, sexual, psychological, emotional and spiritual); relationships between grandparents, parents and children; and between spouses; and societies.

One thing that disturbed me is the abuse children use against each other (buuryaqab, kintirley). As if being created with a part by God is something to be ashamed of and needs correcting. A miasma has been brought about where children are sometimes eager to endure life threatening procedures. This is accomplished by giving children gifts, showering them with praise (you are now a man/ woman). Telling the children geesi ba tahay. And of course not allowing the children to witness an actual procedure. Older children look down on the younger ones and call them dirty (Xalaaleyn baad u baahan tihiin). Cunugkasta oo dhashta waa Muslim.

I read about a Somali religious leader who started to question FGM. His wife had had many health problems. She also lost 5 babies in a row. He now campaigns against FGM. How long must babies, and children as well as men and women continue to suffer? Education is necessary for the younger generation. I think the individual must be given the facts about their body (the anatomy and function) and be told the truth, that the Quran says nothing about either FGM or MGM. Genital mutilation predates Islam.

Having an open mind presents the individual with the possibility that their long held values could be harmful. Then one can connect the dots and look at the issue from all angles. For example, the increasing number of men in the United States taking medications for erectile dysfunction and premature ejaculation. Note: There could be diseases that cause these conditions such as diabetes. Nonetheless, the mutilating of the prepuce' effect on sexual performance must be investigated. There are men who buy penis pumps, and take supplements and medication. Some of these men have their baby boys mutilated ("so he can look like daddy"). The harm that is being inflicted doesn't register because this trauma was performed on them as babies. They never knew their organ any other way. They have gotten used to the pain and discomfort of underwear rubbing against the exposed glans. They don't realize that the satisfaction they derive from intercourse is probably 80% less than had they been intact. So the cycle continues.

People must be asked do you to want feel less pleasure? By cutting the most sensitive parts of the genitalia, it is a fact that sensitivity will be drastically reduced. It is heartening to hear that some of the Somali girls born in the West are left intact. I pray for children everywhere.

Male Genital Mutilation is a difficult subject to discuss with people. Part of the reason for that is because it has been routinely done to babies and children without their consent. Most people are not aware of the harms associated with this ritual. To be sure, some parents are instinctively aware of the severe short-term pain that the baby or child goes through (especially when no anesthesia is used), the difficulty of urination, diaper/ clothing rubbing against a raw wound that was meant to be an internal organ. They are not, however, aware of the long-term effects. There is difficulty in talking about Genital Mutilation, but the decision to mutilate another's body comes much easier for some.

Is genital mutilation harmful? One need only watch videos of babies restrained and mutilated without anesthesia to know that this ritual is extremely painful. There could be therapeutic and emergency reasons for male circumcision which I am not against. I have no problem with an adult male wanting to mutilate his own body provided he is given information regarding the anatomy and functions of the prepuce, the side effects of the procedure, counselling, and alternative therapeutic choices; as well as anesthesia and pain relief during and after the procedure.

Are there adverse effects of MGM? The entire procedure is an adverse effect as we shall see:

In Male Genital Mutilation a healthy part of the penis is mutilated forever changing the way the organ was designed to work by The Most Merciful. The Prepuce protects the glans and the urethral opening when not erect. The prepuce is double layered, having inner and outer skin which stretches during erection. The inner and outer prepuce accounts for 50% of penile shaft skin. The prepuce is highly innervated and contains blood vessels, muscle tissue and mucosa covering the glans thus providing protection. The Ridged Band which is between the inner and outer prepuce contains specialized nerve endings.

The Frenular artery is severed leading to less oxygen and blood flow to the penis. Tens of thousands of specialized nerve endings are severed. The mucosal protection and lubrication provided by the inner prepuce is no more. Subsequently, the glans becomes dry and rough to protect itself from chafing as a result of coming into contact with the diaper, clothing, etc.. The preputial stretching during erection and the self lubricating gliding action (during intercourse) is lost. Five of the most sensitive parts of the penis are ablated. This desensitizes the penis greatly.


"In conclusion, circumcision removes the most sensitive parts of the penis and decreases the fine-touch pressure sensitivity of glans penis. The most sensitive regions in the uncircumcised penis are those parts ablated by circumcision. When compared to the most sensitive area of the circumcised penis, several locations on the uncircumcised penis (the rim of the preputial orifice, dorsal and ventral, the frenulum near the ridged band, and the frenulum at the muco-cutaneous junction) that are missing from the circumcised penis were significantly more sensitive".
